Statement of Disability - Thomas A. Moore
The above letter is a written statement by commanding officer of Co. K stating that "in the line of his duty being in action with the enemy at Helena Ark on 4th July 1863" that Thomas had received a gunshot wound which fractured his skull "carrying away a great portion of the frontal bone" which resulted in dizziness and inability to perform physical or mental labor.

Left for dead on the battlefield, a soldier found a "whisper of a breath" in Tom and he was taken to a Memphis hospital where he was operated on and remained until his discharge in December.
