To J. H. Terry,
Whereas all the proceedings necessary to entitle Lucy J. Williams to be admitted into the Tennessee Hospital for the insane as a non-paying patient have been had according to law; you are hereby required forthwith to take said person and convey her to said Hospital and after executing this warrant make due return of the same to this office.
Witness my hand and Seal of office this the 17th day of May 1889.

The undersigned a citizen of said State and County hereby states that Lucy F. Williams is insane; her insanity is of recent date and that her being at large is dangerous to herself and her family and the community; that she is in needy circumstances and has legal settlement in said County and is citizern of the State of Tennnessee.  These facts can be proved by Ed. S. Carr M. D. and W. H. Cox.  This April 23rd 1889.

W. H. Cox states that he is well acquainted with Lucy F. Williams; has known her from childhood and that she is now dangerously insane and in needy circumstances and she is a citizen of Sumner County Tennessee.

I Ed S. Carr a practicing physician of the County of Sumner State of Tennessee hereby certify that I have personally examined Lucy F. Williams alleged to be insane and pronounce her so; is 35 years of age, is the subject of no infectious disorders; the disease is of recent date; she is not subject of Epilepsy; has attempted violence on her family; the medical treatment has been tonics and nervous sedatives.

I the undersigned Justice of the Peace in & for the county aforesaid hereby certify that I have held an inquest on Lucy F. Williams of said county, a person alleged to be insane, according to law I am satisfied that she is insane on competent medical testimony, that she is a fit subject to be sent to the state hospital for the insane to undergo treatment therein as an object of bounty of the state.  I am further satisfied that her being at large is injurious to herself and disadvantageous to the Community.
