The undersigned a citizen of said state and county hereby states that Miss Johnetta K. Cantrell is insane, her insanity is of more than two years duration or that her being at large is dangerous to the safety of the community that she is in needy circumstances and has legal settlements in the said county and is a citizen of the state of Tennessee.

I J. H. Joyner a practicing physician of the county of Sumner State of Tennessee hereby certify that I have this day personally examined Johnetta K. Cantrell alleged to be insane and pronounced her so.  She is forty  (40) years of age, is a subject of no infectious disorder.  The disease is of two (2) years duration (mentally).  She has had epilepsy for 28 or 30 years, she has attempted to commit violence on Mrs. Kate Cantrell about 2 or 3 years ago evincing a mental dethronement also at various times since that on others.  More especially two weeks ago, when she attempted to commit violence on Allice & Kittie Cantrell & declared she would kill them.  J. The said Johnetta Cantrell was treated by Dr. James Franklin 25 years ago with bromide of Potash & the usual treatment for epilepsy with the exception of Sultrate of Silver which Dr. J. R. Kirkpatrick gave her until it discolored her skin very dark this was a few years after Dr. Franklin treated her.  I have been called to see her repeatedly since that time giving her the usual treatment for epilepsy.
I have discovered for several years her mind was being impaired for about two years I have considered her partially insane.

I the undersigned Justice of the Peace in & for the county aforesaid hereby certify that I have held an inquest on Johnetta K. Cantrell of said county, a person alleged to be insane, according to law I am satisfied that she is insane on competent medical testimony, that she is a fit subject to be sent to the state hospital for the insane to undergo treatment therein as an object of bounty of the state.  I am further satisfied that her being at large is injurious to herself and disadvantageous to the Community.

To James M. Cantrell, whereas all the proceedings necessary to entitle Johnetta Cantrell to be admitted into the Tennessee Hospital for the insane as a non paying patient have been had according to law, you are hereby required forthwith to take said person and convey her to said Hospital and after executing the warrant make due return of the same to this office.
Witness my hand and seal of office this 23rd day of December 1890.
