To W. F. Cline,
Whereas all the proceedings necessary to entitle Mrs. Mary Ann Calvert to be admitted into the Tennessee Hospital for the insane, as a non-paying patient, have been had, according to law, you are hereby required forthwith to take said person and convey her to said hospital and after executing this warrant make due return of the same to this office.
Witness my hand at office this 4th day of Febry, 1893.

The undersigned a citizen of said state and county hereby states that Mrs. Mary Ann Calvert is insane, her insanity is of less than two years duration and that her being at large is dangerous to the safety of the community that she is in needy circumstances and her legal settlement in the said county and is a citizen of the state of Tenn.  These facts can be proved by A. A. Black and J. A. Hannah.

I, R. Durham a practicing physician of the County of Sumner State of Tennessee hereby certify that I have this day personally examined Mrs. Maryann Calvert alleged to be Insane and pronounce her so.  She is 48 years of age, is the subject of no infectious disorder, the disease is of less than two years duration.  She is not the subject of epilepsy.  She has attempted to commit violence on her self and others.  The Medical treatment has been ______ Nothing.  Sworn to and subscribed before me on this January 31, 1898.  R. State of Tenn, Sumner County.  I the undersigned Justice of the Peace in and for the County aforesaid hereby certify that I have held an Inquest on Mrs. Maryann Calvert of said county a person alleged to be Insane according to Law I am satisfied that she is Insane on competent Medical testimony that she is a fit subject to be sent to the State Hospital for the Insane to undergo treatment therein as an object of Bounty of the State.  I am further satisfied that her being at large is injurious to her self and disadvantageous to the Community.
