Death Records and Certificates

Some records will require the Adobe Reader in order to read them. If you do not have it you can get it at the Adobe Download page. I have a lot of records and death certificates to scan. So bookmark my website to return. Some of these are not from our lines. You will know the difference if you are a researcher. There are times that I collected information and later found they were not related to our lines. I will include those files here in order to help others. Sometimes seemingly unrelated individuals end up actually being related when further research is done. For that reason I feel it important to list all files I have collected during the years. If you are able to find links in your family that my records helped with, please, let me know.

Records listed in alphabetical order for last name. First names may not necessarily be in alphabetical order under the last name.
I add them as I scan them so most of the time I will add new items at the end of their surname files.

John Daniel 1913-2001. Obituary from Merced Sun Star, Merced, CA.

John Daniel death certificate
John had two middle names that his mother gave him. However, when he applied for the
delayed birth certificate(see on births page), when he was 46 years old,
he only used one of his middle names. He had been told not to use both.
Maybe that is why only one is on his death certificate. I did give them both.
The spelling of his name that I saw the most was John Buford David Daniel.
I know that he would want his entire name remembered.
I gave my youngest son the middle name David after my dad.

Flossie Dimple Stephens (1910-2004) Death Certificate.

Obituary: Flossie Dimple Stephens 1910-2004

Frank Stephens 1911.
9 month old son of John and Edith(Chapman/Chatman)Stephens.
My grandfather always told the story of a baby girl also around this time, that did not die.
Another family member doing genealogy found a Frankie Stephens that was believed to be a girl.
That name may be wrong since this death certificate is for a male child.
My grandfather spent many years searching for his baby sister, who was cared for
by an unknown family, after his mother passed away. When his father went back
for her the family had moved away.

Gladys Estella Stephens age 15
Wayne County, Greenville, Missouri. Daughter of Finis A. and Mary(Wright)Stephens.

James C. Stephens 1828-1910.
Lewis County, Missouri.

James Woodrow Stephens age 1 1/2
Dunklin County, Missouri. 1918.

John Samuel Stephens 1867-1947.
Dunklin County, Missouri. Father of my grandfather Charley Wesley Stephens.
Last name spelled wrong on death certificate. It was spelled Stevens instead of Stephens.
Took longer to find because of the mis-spelling. This has been something
I have run into many times in family research.

John Stevens 1901-1910.
9 year old son of Josiah Stephens. The death certificate must have
spelled the child's last name wrong since the father's last name was Stephens.

John Wesley Stephens 1860-1954.
Wayne County, Missouri. Haven't discovered possible relationship. Name Wesley that was given as middle name,
to my grandfather, was after an unknown (to me) relative.

Kittie Stephens 1877-1911.

Lessie Stephens
This is an unknown Lessie Stephens. She died in 1920 of a gunshot wound to the chest at age 22.
Found this death certificate while looking for the death certificate of Lessie (Foster) Stephens that was married to Charley Stephens.
The other Lessie married in 1922. The marriage license is in the marriage records page.

Robert Stephens about age 50. Suicide. 1912.
St. Louis, Missouri. Commited suicide by drinking Carbolic Acid. This Stephens commited suicide in the same town as
the murder suicide seven months later of a couple by the same last name of Stephens
(see death certificates of Ada Stephens and William Stephens).

W.A. Stephens 1844-1908.
I transcribed this record below the original because it was hard to read.

Wesley Arnel Stephens Jr. Obituary. 1948-1981.
I want to thank Wesley Jr's sister Bonnie Odgers for sending me all of the information on her brother that she had saved.
See below for newspaper article, death certificate and police report that Bonnie supplied for this page.

William A. Stephens 1871-1912. Suicide.
St. Louis, Missouri. Died of gunshot wound to head. Listed as Suicide November 1, 1912.
Wife Ada, listed above, died same day and her death was classified as Homicide.

William E. Stephens 1842-1916.

Willie Stephens 1907-1928. Female.

Zelma Juan Stephens 1936-1937
She was called Juan, which was her middle name. Her death certificate lists her first name
and middle initial, probably because the one filling out the form did not know how to spell Juan.
It is pronounced Ja-One. She was the last child that Charley and Flossie had.
Juan's oldest brother, who was almost nine at the time of her death, named his youngest
daughter Mary Juan and also called her by her middle name. Most of the family still
calls her Juan, however to many of her friends and professionally, she goes by Mary Case.

WHEELER
Deaths Certificates, obituaries and death records.

Olive Wheeler and Philander Wheeler
I also included this under Otto above and under Wheeler because Olive was a Wheeler for many years.
Her last name was Terry when she was married to Parshall Terry.

WILLIS
Deaths Certificates, obituaries and death records.

Richard Eugene Willis 1946-2008
Death Certificates only have the information given by family members present.
If the information is not accurate later generations may be records that information.
With Richard's death certificate we find that his mother's maiden name is wrong. It says Daniels.
There was no s on her last name. It was Daniel. Other inaccuracies I have found here are;
Elmer Willis was born in Oklahoma and Minnie Daniel was born in Tennessee.
