On the 22nd day of May, A.D. one thousand nine hundred and twelve,
personally appeared before me, a County Court Clerk within and for the
county and State aforesaid, Louis A Belcher, who, being duly sworn according
to law, declares that he is 76 years of age, and a resident of county of
Grant, State of Kentucky; and that he is the identical person who was
enrolled at Falmouth Ky under the name of Louis A Belcher, on the 19th day
of Oct, 1861, as a private, in Company ?D? 18th Kentucky Volunteer Infantry
in the service of the United States, in the Civil war, and was honorably
discharged at Louisville, on the 20th day of June, 1865.
That his personal description at enlistment was as follows: Height, 5 feet
5 inches; complexion, Light; color of eyes, gray; color of hair, Light; that
his occupation was farming; that he was born in the year, 1835 in West
Virginia.
Kenton County Ky, moved to Grant County Ky about 4 years after war.
That he is a pensioner under certificate No. 1009684.
That his post-office address is Williamstown, county of Grant, State of
Kentucky.
