STATE OF SOUTH CAROLINA STATE HOSPITAL FORM FOR THE COMMITMENT OF THE INSANE, PRESCRIBED BY THE BOARD OF REGENTS OF THE SOUTH CAROLINA STATE HOSPITAL PURSUANT TO THE PROVISIONS OF LAW Application
Robt. W. Jones IN THE MATTER OF AN APPLICATION FOR THE COMMITMENT OF Milton T. Jones AN ALLEGED INSANE PERSON
To the Honorable Fannie C. Scott, Judge of Probate, of the County of Greenville, State of South Carolina, the Petition of Robt. W. Jones respectfully shows: 1) That he is a resident of Greenville in the County of Greenville and that he is Son of Milton T. Jones, the alleged insane person. 2) That the alleged insane person is now at the home of City Hospital, Greenville, County of Greenville, and is a legal resident of South Carolina. 3) That the facts upon which the application is based are as follows: Mind comes and goes; takes wild spells so that family can not do anything with him; goes off, and cannot be located. 4) That he verily believes it to be for the best interest of the said alleged insane person that an order be granted directing his commitment to the State Hospital. 5) Upon information and belief that the said Milton T. Jones herein mentioned is not under a criminal charge or indictment. 6) Upon information and belief that the schedule of property listed below is correct. Property owned by Milton T. Jones (Lines 31 through 42, soliciting data for assets in real property and personal property, and debts for both husband and wife and children, all completed with answer None.) 7) That the nearest relative or friend with whom the superintendent may correspond relative to the condition of the patient is Robt. W. Jones, Son, 103 Buncomb Road, American Spinning Co., Greenville, S. C. 8) That the patient can be delivered to a representative of the hospital at will be at City Hospital.
Wherefore, upon the foregoing facts and the medical certificate of insanity, your petitioner prays that an order be granted, adjudging the said alleged insane person to be insane and committing his (sic) to the State Hospital for care and treatment.

STATE OF SOUTH CAROLINA COUNTY OF Greenville Robt. W. Jones, being duly sworn, deposes and says that he has read the foregoing petition and knows the contents thereof, and that the same is true to the knowledge of the deponent, except as to the matters herein stated to be alleged on information and belief, and as to those matters he believes it to be true.

Jones is insane, and a proper subject for admission to the South Carolina State Hospital, you are hereby commended to examine, without delay, the said Milton T. Jones as to his alleged insanity, and report result of said examination, in writing, to me.
