The Death Certificare Index has been reformatted. I never realized that I would have so many death certificates, funeral home cards, death notices and obituaries on my site. There is now a main ABC index. Clicking on the letter of the surname that you wish to view will bring up another index listing all surnames beginning with that letter. Click on the surname you are looking for. From there, you will have a listing of all death certificates, funeral home cards, obituaries and death notices for that particular surname to choose from. In case of women with multiple mairrages, they have been listed with the surname listed on the document. Notations have been made to their multiple marriages/surnames.

I am able to get death certificates for the state of Kentucky from 1911 - 1956 (ONLY). Death certificates will not have a state of Kentucky seal on them, but work well for genealogy purposes. The cost to obtain the death certificates, which includes the photocopying, envelopes, and postage is $1.50 per death certificate.

The death certificate information needed is as follows:
(The more information provided the better the chances are of finding them.
Unfortunately, not all individuals have a death certificate.)

* Name of deceased
* Date of death
* County of death
* Parents names (if known)
* Spouses name (if known and applicable)
* Date of birth (if known)

The death certificates can be looked up either by you or by myself.
If you are going to look up the death certificates, the following information is needed:

Name of Deceased
Death Certificate Number
**DEATH VOLUME
(**This is different than Volume and makes a difference when looking up certificates.
Sometimes the death certificate is filed in a different year than when deceased.)

Death certificates may be looked up on the
KENTUCKY DEATH INDEX.
