Kentucky Deaths 1911-1999 I only have the Kentucky Death INDEX from 1911-1999 which provides you with the name, date of death, county death was in, county of residence, the Certificate number and Volume number (which you need to order the entire death certificate) and the age at death and the year the death was registered. The Parents are NOT listed on the INDEX but possibly may be listed on the certificate. You can go to www.google.com and type in Kentucky Vital Statistics and you should be able to find where you order them. If the information is not there, please know that I tried various spellings of the names given and the entire State as well.
I am in Florida and do not have access to any other resources except what is listed.

The cost to obtain the death certificates, which includes the photocopying, envelopes, and postage is $1.50 per death certificate.
