my disability has greatly increased since last examination the ankle has become
so very week and the joints slip and creak when I walk. My foot is even
worse than the ankle the foot is almost stiff and I have not good use
at all of my foot-it was broke across the instep, and the break of instep looses
me the power of the great toe it is perfectly stiff. Also my left
foot and leg to the knee is effected with rheumatism or something
similar which causes pain.
I ask your honor that I be ordered for an examination to Cambridge

We are residens of Belmont County State of Ohio we are personally acquainted with David H. Heskett who was a private in Company G of the 174 OVI we were members of Company G. of the 174 and we were present on the train at the time of the collision in which he received his injuries which accident occurred near Greensborough N.C. on or about the 11th day of May 1865 the said fracture and dislocation being in the left ankle. I have known David H. Heskett since the close of the war; late rebellion in the United States, and treated him in the year of 1873 for disease of the lungs; I knew of him having a diseased or crippled ankle , but did not treat him for it; and it had become of such a chronic nature that medical treatment could not have affected a cure.
and whose age is now 59 years, being duly sworn, says that he has been acquainted with David H. Heskett a private in Co. G. 174 Regiment O.V.I. prio to his enlistment in said Co. & regiment and in fact from his boyhood up to said time, that said David H. Heskett was free from any ailment of ankle or otherwise before enlistment, that he knows the above facts from personal knowledge and intimate acquaintance being a near neighbor of his, he further says that he has known him almost constantly since his discharge from the army, that he has complained of injured ankle incured while in the army. He further states that said David H. Heskett is a man of temperate habits and a good upright citizen; I further swear that I am not interested in this claim for pension.

G. 174 Regiment O.V.I. for 30 years that he was intimately acquainted with him at the time he enlisted in the army and for 13 years previous to his enlistment; that he believes said David H. Heskett was free from any disease or ailment of left ankle at the time and before his said enlistment. The said Richard A. Corbin further says that he, said witness, enlisted as a private in Comp H. 180 O.V.I. 23rd army corps; that on or about May 20th 1863 at Charlott N.C. he seen said David H. Heskett while he was carried from depot to hospital on strecher and afterwards seen him in hospital while he was crippled and suffering from injury of left ankle; which he learned had happened some 8 or ten days before 20th day of May last aforesaid; he further says he has known David H. Heskett continously since his dischargefrom said army up to the present time and that said David H. Heskett has ever since complained of said diseased or crippled left ankle. He further says that David H. Heskett is a man of temperate habits and a good upright citizen. I further sear that I am not interested in this claim for pension.
John J. Heskett whose Post Office address is Bethesda County of Belmont State of Ohio and whose age is now 55 years; being my me first duly sworn according to Law; says that he is and has been acquainted with David H. Heskett late a private in Co. G. 174 regt O.V.I. Prior to his enlistment in said company and regiment and in fact from his birth up ot the present date; That said David H. Heskett was as he believes free from any disease or ailment of left ankle, before said enlistment, that since his discharge and return from the army he has a diseased or crippled ankle; which incurred as he is informed while in the service and in the line of his duty; That said David H. Heskett has ever since his discharge or his return home from the army continously each and every year complained of said ankle; that its impairment rendered him incapable of performing manual labor from one fourth to one half less than before the time he went into the army;the above facts and infirmation I have obtained from personal knowledge and intimate acquaintance; with said David H. Heskett; I further know him to be a man of temperate habits; I further swear that I am not interested in this claim for pension.
I have known David H. Heskett since 1860; that prior to his enlistement in the war of the Rebellion he was sound in health, free from any disease or ailment of left ankle and foot. I also seen said David H. Heskett at the time he returned home from the army, immediately after his discharge; which time refered to was about the 10th day of July 1865. I remember that said David H. Heskett was then walking upon crutches and complained of cripled ankle and foot. I have also known said David H. Heskett from the date last aforesaid, to the present time, except within the last two years he has been absent in the state of Iowa; but has made one visit back since his sojourn in Iowa; said David H. Heskett has ever since complained of his ankle ad foot and as far as I know from his disability would say it rendered him unfit to perform manuel labor one half on account of said disability. I further know him to be a man of temperate habits.

good standing at Demorect, Ga. make the following affidavit.
To whom it may concern.
In examining David H. Heskett I find that he is now suffering with his left foot
and ankle due to a crushing and dislocation of said foot and ankle between
two car while in active service during civil war in the Federal army. The
above condition has existed to a greater or less extent since he was
injured, but as he grows older it is becoming more apparent. There is
an ankylris between the internal metatarsal and interanl cunciform bones of the left
foot and between the interanl cunciform and the navicular bone. When he
walks the distal end of the internal metatarsal presses hard against--
the ground making a perceptible limp, unless he constantly bears it
in mind and overcomes it by conscious effort. Mr. Heskett is a farmer
and he has to depend altogether upon the service of others to do his
manaul labor. Walking over rough ground causes great weariness. Recently
he has been having much pain across the lumbar plexus with deep pain
in left hip. I believe this is due to constant strain of the muscles
of leg in trying to shield the injured ankle and foot. I take great
pleasure recommending that he receive a pension in accord with his
disability.
Mr. David H. Heskett, whom I know personally and with whom I come into almost daily contact, needs a caretaker with him at all times as he is constantly suffering from serious back trouble which renders him absolutely unfit for any work, even of the mildest form. Also, Mr. Heskett is subject to dizzy spells which strike him instantly without the least notice, at which times he falls completely helpless where ever he may be. Mr. Heskett is also crippled from a crushed left ankle, received during the Civil War.
Mr. Hesketts trouble is general breakdown in health as result of old age. Suffers with back trouble, dizziness for which he needs an attendant.

I am writing for my Father, David H. Heskett, now in the Erie County
Soldiers Home. I was over there to see him today. He is not able to write,
but wishes me to answer your enclosed letter. He wishes his pension check
to continue coming to my residence, at 2539 Auburn AVe., Toledo, Ohio. I am
his own son, and his only near relative. He will not doubt return to
my home if his health permits. If you should wish to investigate my
responsibility, I am letter carrier, No. 5. Toledo, Ohio; and having served
here for thirty years, the Toledo Post Master could not doubt give you
the desired information. I have been depositing his spare money in the
bank for him, and buying his supplies.
