Third Saturday in August 1834 the church met in conference and after prayer proceeded to business.

1^st Inquired for fellowship.

2^nd Opened a door for reception of members and received the following by experience. Viz Lucy Cheatham, Elizabeth Marshall, Mary Ann Cheatham and Rachel Porter by letter Constituted in the Church.

3^rd The Church agrees to send a petition any letter and one dollar to the association to be held on War Eagle in Washington County.

4^th Appointed brother Samuel Bristow to bear our presents to the association

5^th The Church agrees to be called Crooked Creek Church
