- SSA's info on requesting copy of SS-5, social security application - kinda hard to read, but the info is there. Isn't that just like the gov't? ;-) You can order it online at https://s044a90.ssa.gov/apps7/efoiassa/internet/SSA711.jsp If the person is deceased, e.g. on the SSDI, you can request a copy of anyone's SS-5. Cost is $7 with SSN and $16.50 without.
