Unofficially as the "Death Plaque" and officially as the "Next of Kin Memorial Plaque". Each was individually cast. Over 1.3 million were produced. (Received from Pat Hoppe. She understood that the Medallion was cast from a shell casing.)

Provided by Pat Hoppe. She received it from "Uncle Al" (Basil) Glynn in the 1970's. The source of publication is not known.
