"A" 1st Tenn Mtd. Inft. in the War of the Rebellion, and served at least ninety days and was honorably discharged at Nashville, Tenn. on the __(blank)__ day of April, 1865. That he is now unable to earn a support by reason of Catarrh on head, rheumatism affecting shoulders, breast, knees and legs, by exposure while on duty. He agrees to pay his attorney 10.00 for prosecuting his claim. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent, that he has never applied for pension under application No. __(blank)__; that he is a pensioner under certificate No. __(blank)__. She went back and they told her that he died on the 13th day of April 1863 and she has not never seen or heard of him since and she has good reasons to know that he wer dead for he wer allmost dead the last time she saw him in Knoxville and she noes of no other Wittness that she can prove his death. "A" 1st Tenn. Vol. Inft. and served at least ninety days in the late War of the Rebellion, in the service of the United States, and was HONORABLY DISCHARGED in 1864 and died December 20th 1899.
