The following form is designed to help you organize your family tree information. Please use full names and maiden names whenever they are known. Leave blank any boxes which you do not know. If there is more than one marriage use a separate form for each spouse and the children of that spouse. This form may be used repeatedly for other generations, other marriages, etc.

You may use this form in several ways:

• Print this form, fill in the blanks, and either mail it to me (I'll reimburse postage) or scan it and e-mail me the result.

• If you use Netscape (NOT Internet Explorer) you can fill in the blanks and "submit". Just be sure to "submit" and "reset" before starting each new family. (Only Netscape will actually send.)
