I certify, on honor, that Anthony Tenbush a Private of Captain J. B. Ayres Company B of the 14th Regiment of Cavalry Volunteers, of the State of New York, born in Germany, State of ---------, aged 21 years; 5 feet 7 inches high; fair complexion, blue eyes, light hair, and by occupation a nurseryman, having joined the company on its original organization at N. Y. City, and enrolled in it at the muster into the service of the United States at Rikers Island, on the third day of February, 1864, (or was mustered in service as a recruit, by Captain Bristol, at Rochester, N. Y., on the 18th day of November, 1862, or was drafted and mustered into the service of the United States from the --------Enrollment District of the State of ------------, at -------------- on the ----day of ---, 186---,) to serve in the Regiment, for the term of three years: and having served Honestly and Faithfully with his Company in the field to the present date, is now entitled to a Discharge by reason of Death - "died Sept 9/63 at Marine Hospl, New Orleans, La., of Typhoid fever.

The said Anthony Tenbush was last paid by Paymaster Major Hoyt to include the 30th day of April, 1863, and has pay due him from that time to the present date; he is entitled to pay and subsistence for Traveling to place of enrollment, and whatever other allowances are authorized to volunteer soldiers, drafted men, or militia, so discharged.  He has received from the United States Clothing amounting to Forty-three 80/100 dollars, since the ------day of his Enlistment. 186_, when his clothing account was last settled.  He has received from the United States none...dollars advanced Bounty.

There is to be stopped from him, on account of the State of --------or other authorities, for Clothing, &c., received on entering service, none dollars; and for other stoppages, viz: none.

He has been furnished with Transportation in kind from the place of his discharge to ---------and he has been subsisted for traveling to his place of enrollment, up to the -----------, 186_.

He is indebted to -------------, Sutler, --------dollars

He is indebted to --------, Laundress, --------dollars.

Given in duplicate, at Highland Stockade in Baton Rouge this 29th day of July 1864.
